A Non-Profit Organization would complete this form to request changes on their account. Step 1: Download Business Maintenance Form Step 2: Fill out the contact section of the form, fill out company name, contact email, phone number, billing address, and billing/control account number. Step 3: Choose the appropriate option. If you are looking to close the full account, select close account. If you are looking to close only specific cards, state close cards ending in and list the last 4 digits of the cards you are closing. Step 4: Sign the bottom of the form.
